I was hurt at work and went to the doctor same day. I received a letter from the workers comp insurance company. It has an injured worker questionnaire form they want me to fill out and send back. There is a spot on the form they they want me to write down my social security number. My question is do I have to give it to them and if I refuse could they deny my claim and not pay the doctor bill? I'm worried about it ending up in the wrong hands and identify theft. Every day you hear about a company's servers getting hacked and people's data being stolen. Located in Minnesota
